C.H.A CPR Participant Liability Waiver

PLEASE READ CAREFULLY BEFORE REGISTERING OR PARTICIPATING IN TRAINING.

In consideration of participating in CPR, AED, First Aid, or related emergency care training provided by C.H.A CPR, I (the undersigned participant) acknowledge, understand, and agree to the following terms:

1. Acknowledgment of Physical Risks

I understand that participating in CPR, AED, and First Aid training involves physical exertion, including continuous chest compressions, kneeling on the floor, lifting or moving manikins, and practicing physical rescue maneuvers. I acknowledge that these activities carry inherent risks of physical strain, muscle soreness, minor injuries, or other unforeseen physical events.

2. Voluntary Participation & Medical Fitness

I affirm that my participation in this training course is entirely voluntary. I certify that I am physically capable of participating in the hands-on skill practice required for certification. I agree to notify the instructor immediately if I experience any pain, discomfort, or physical distress during the session, and I am free to stop or withdraw at any time.

3. Release and Waiver of Liability

To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue CHA CPR, its owners, agents, and instructors , from any and all liability, claims, demands, or causes of action arising out of any physical injury, loss, or damage that I may sustain while participating in this training class or using its equipment.

4. Educational Disclaimer & Limitation of Liability

I understand that this training course is designed to teach standard emergency care guidelines (following current OSHA, AHA, and ECC recommendations). I acknowledge that CHA CPR and its instructors provide instruction and issue course completion documentation for educational purposes, but do not guarantee, warrant, or assume liability for how I or any third party perform emergency medical care or first aid in real-world situations following this training.
